In a big bowl, toss ground beef, breadcrumbs, milk, ketchup, egg, garlic powder, onion powder, Worcestershire sauce, salt, and pepper. Mix until just incorporated; steer clear of overmixing to maintain tender meatloaf.
On a parchment paper-lined baking sheet, form half of the meat mixture into a loaf. Make a central well and load it with mac and cheese. Top the leftover meat mixture such that the mac and cheese is encased on the edges.
